Did your request even got accepted? If you did /requestadunmute and it didn't got accepted by an administrator, you won't be fined plus the mute will remain.
In order to proceed with the mute, it requires an administrator's approval which leads to the mute fees and un-mute from the advert system, otherwise, nothing changes.

When you're in-game, use /reportbug. I'd suggest speaking to a developer regarding this bug as it could be specifically only happening for your account and not everyone. When you're next in-game, do /requestadunmute, when it gets accepted, take a screenshot (F8) and then relog and take another screenshot of you still being muted. You can then use this evidence to verify the bug for a developer and also get any money that has been lost due to this bug refunded by posting a GRR here.
